About us
The Vision for Crafter Health is to be at the forefront of General Practice by innovatively delivering exceptional care today, and into the future. With an emphasis on traditional family medicine, high quality medical care is provided to all patients, their families, and the local Mitcham Community.
Crafter Health is a fully accredited practice, which means there has been a rigorous process of assessment against the Standards set by the Royal Australian College of General Practitioners. These Standards govern all aspects of medical practice including the policies, procedures, infection control, computer systems, quality of medical care, continuing education, communication and more.

Your first appointment
Please arrive 15 minutes early for your first appointment to give you time to fill in patient registration paperwork. Alternatively, you may wish to complete an electronic Patient Registration Form prior to your appointment. This will be sent by SMS prior to your first appointment. If you wish to request a transfer of your medical records from your previous doctor, you can also download and complete the Transfer of Medical Records form and submit it to the reception desk prior to your appointment.
Reception staff may assist you in registering for My Health Record, a secure online summary of your health information. Before signing the consent to register, please visit the My Health Record page to read further information regarding your privacy and security here.
Booking terms and conditions
All new patient bookings incur a $110 non-refundable deposit (plus card processing fee) to secure your appointment.
This deposit will be credited toward your total fee on the day. Please note that the actual cost of your appointment may be higher depending on the consultation.
Cancellations made less than 24 hours before your appointment will forfeit the deposit. Cancellations made more than 24 hours in advance can be rescheduled without penalty.
Interpreter and other communication services
When booking, please let us know if you require an interpreter or other communication service. Crafter Health provides for the communication needs of patients who are not proficient in the language of our clinical team, or for those who have communication support needs.
